NEWBORN MODEL CALL APPLICATION

The purpose of this form is to determine if your BABY would be suitable participants for the current creative project I'm working on.
Please be as thorough in your descriptions as possible. Please note that the final photos may or may not be posted on the Prop Vendors website.
This model call is a session for BABY ONLY. There will be NO FAMILY SHOTS taken during the session -- but there is an option to upgrade to get family shot done. (Please mention on the form below if interested to upgrade)

What we're looking for:
- 1 Baby Boy and 1 Baby Girl born after January 15, 2026
What are you going to get?
- at least 5 fully edited BABY images

BY SUBMITTING THIS FORM IT INDICATES THAT YOU UNDERSTAND THAT IN ORDER TO PARTICIPATE IN A CREATIVE SESSION, YOU MUST SIGN A MINOR MODEL RELEASE WHICH GRANTS THE PHOTOGRAPHER THE RIGHT TO USE AND PUBLISH PHOTOS AND VIDEO CREATED DURING THE SESSION FOR ANY ADVERTISING, DISPLAY, PUBLICATION, MARKETING, COMPETITION AND/OR OTHER STUDIO PURPOSES.
